Duplication Of Digits
Duplication Of Digits
It is a congenital defect of digits. Popularly known as polydactyly or supernumerary digits. Along with syndactyly, it is the most common congenital defect of limbs. Normally a person has five digits in each limb.
In polydactyly, person has more than five digits either complete or incomplete. Here is a child who has duplication of thumb and great toe in all four limbs. Removal of one extra digit is not a major surgical issue. But restoration of anatomic and functional duplicated digit is very important. It requires complete anatomical and functional knowledge of that particular digit. Assessment of both the digits are very important e.g.. Which one is to be kept and which one to be discarded, its muscular attachments, neuro-vuscular distribution-all are to be assesed properly before going for surgery.

Duplication Of Thumb
In this case i decided preoperatively to discard the outer thumb because, it was anatomically and functionally weaker than the inner one. On opening the outer thumb i found that all thenar muscles were attached to the outer thumb. Neuro vuscular distribution were equally divided to both thumbs. All long tendons were attached to inner thumb. I had tranferred all thenar muscles to inner thumb and amputated the outer thumb skeleton. And refashioned the skin and soft tissue to give a normal looking hand.

Duplication Of Great Toe
Here i decided preoperatively to remove the inner toe due to same reason. On opening the inner toe, i found that all long tendons, both flexor and extensor, were attached to inner toe. Short muscles and neuro-vuscular distribution were with the outer one. I transfered all the long tendons to the outer toe and amputated the inner one. Then i refashioned the skin and soft tissue to give a normal looking great toe.

Handwashing week
Recently we organized Handwashing week from 15/05/2011 till 21/05/2011. This endeavour was taken to spread the awareness about the importance of handwashing among healthcare workers. It has been well known that this much important and inexpensive habit helps in reducing the nosocomial infections and hence reduce the cost of treatment, complications and hospital stay of patients, and thus significantly improve the overall outcome of treatment. It is important that every healthcare professional inculcate this habit and follow it religiously to make it a success. This was followed by an evaluation session to check the efficacy of the programme. The programme was initiated by the infection control committee of the hospital. We propose to hold repeated such programme in the future.
